Notice of Privacy Practices

How we protect your health information, and the rights you have over it.

Effective date: November 2023

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Who we are

Sahara Transportation LLC is a non-emergency medical transportation (NEMT) company based in Kansas City, Missouri. We drive patients to and from medical appointments, dialysis, therapy and hospital discharges, and we bill insurance for those rides.

You can reach us at (816) 714-4823 or infosaharatransport@gmail.com.

What health information we collect

To arrange and bill for your rides, we collect protected health information (PHI). This can include:

  • Your name and phone number
  • Pickup and drop-off addresses
  • Appointment details, such as the date, time and the facility you are visiting
  • Insurance information, such as your plan and member ID
  • The type of service you need, such as wheelchair, ambulatory, dialysis or hospital discharge transport

When you use the ride request form on this website, the details you enter are sent to us by email through a third-party form service. Please do not include diagnoses, medications or medical records in the form. We will discuss anything we need by phone.

How we use your health information

We use your information only as needed to provide and pay for your transportation:

  • Scheduling rides. We use your name, phone number, addresses and appointment details to plan, confirm and complete your trips.
  • Billing insurance. We share the information your insurance plan or Medicaid program needs to approve and pay for your rides.
  • Coordinating with healthcare providers. We may confirm pickup times, appointment times or discharge plans with the facility you are traveling to or from.

Who we share your health information with

  • Insurance payers, including health plans, Medicaid and transportation brokers, to arrange and receive payment for your rides.
  • Healthcare facilities, such as hospitals, dialysis centers and clinics, to coordinate your pickup and drop-off.

We may also share information when the law requires it, for example to respond to a court order, to comply with a government agency investigation, or to prevent a serious threat to someone's health or safety.

We do not sell your health information or use it for marketing. For any use not described in this notice, we will ask for your written permission first. You can take back that permission in writing at any time, except where we have already acted on it.

Your rights

You have the right to:

  • See and get a copy of the health information we hold about you.
  • Ask us to correct (amend) information you believe is wrong or incomplete. If we say no, we will tell you why in writing.
  • Get an accounting of disclosures, a list of certain times we shared your information outside of rides, billing and normal operations.
  • Request restrictions on how we use or share your information. We are not required to agree in every case, but we will consider every request.
  • Request confidential communications, such as asking us to call a different phone number or send mail to a different address.
  • Get a paper copy of this notice at any time, even if you agreed to receive it electronically.
